Review Consent Forms

Often, consent form have health related questions such as “Has this animal been eating and drinking normally?” or “Has this pet been to the vet in the past thirty days?” If that’s the case, then you will need to review the consent forms to make sure they’ve not mentioned anything concerning. To review consent forms:

1. Go to https://clinichq.com/app#/checkout/paperwork
2. Select the Correct Date, click Select All tick and click Print Selected button.
3. Select the Signed button under the Consent Forms section, and click Print Selected Documents button. This will generate a PDF for you to view. It will not print without further steps.
4. Then, scroll through the documents and review the answers to questions. If there’s anything concerning, call the client to discuss.
