10.3.4. SOAP

SOAP notes made quick and easy to help reduce time in front of the computer and focus on the patients in front of you.

S ubjective = Information provided by the patient or owner.
O bjective = Observable and measurable data.
A ssessment = The healthcare provider’s diagnosis or interpretation of the situation.
P lan = The recommended course of action, treatment, or follow-up steps.

Default Normals

The quickest method is to leave all values set to normal and only change them to abnormal if needed. This helps reduce the number of clicks!

If your clinic prefers to not have “normal” pre-selected, you can disable it in Settings > Patient Flow Setup > and untoggle the toggle for “Set exam defaults to ‘normal’. If not enabled, the TPR, Subjective and Objective will not be auto-selected to normal.”
