8.8. The Rabies Certificate

h3. The rabies certificate is a stand alone document that can be used when a stand alone rabies certificate is required.

We do not find using the stand alone rabies certificate to be an efficient use of resources. Instead we suggest building the required rabies fields into your Take Home Paperwork.

The fields required for a rabies certificate in most states are the following.

• Rabies vaccine
• Manufacturer
• Lot Number
• Lot Expiration
• Duration of the Vaccine
• Supervising veterinarian who gave, or supervised, the injection
• Date given
• Owner’s name
• Physical address
• Pet’s name
• Pet description

All of these pieces of information should be included in your visit summary. So, the visit summary does dual duty as a visit summary, but also proof of rabies vaccine, also known as a rabies certificate.
