Paperwork makes the world go round!
Especially for spay/neuter clinics. This section includes a discussion of key paperwork used by most spay/neuter and wellness clinics. Here are the most common types of paperwork you’ll need.

- Consent Forms: Consent forms are used to get permission from pet owners for procedures like sterilization or other treatments. These forms explain the procedure and its risks. By signing, pet owners confirm they understand the information and release the clinic from any liability related to the procedure.
- Treatment Sheets: A treatment sheet is where a physical exam, anesthesia, services, rabies vaccine info, and other notes are included.
- Cage Cards: These are printable cards that you can put on the cage.
- Take Home Paperwork: This is the aftercare paperwork. It includes a summary of the appointment, rabies certificate and aftercare instructions.
- The Medical Record: This is the official medical record for the patient. It includes a summary, a treatment sheet for each visit and any Idexx test results.
- High Risk Waivers: These are waivers you issue to clients for high risk patients.
- The Rabies Certificate: We’ve included a rabies certificate in HQ but it is our strong recommendation to simply integrate it into your take home paperwork.
- The Spay/Neuter Certificate: In most cases, the Take Home Paper work suffices as a spay/neuter certificate. We’ve included a certificate here but most clinics don’t use it independently.
- The Roster: The roster is a summary of daily appointments. Clinics will use the roster to prepare for their day.
