Email High Risk Waivers

First, it should be noted to use these tools, your clinic must be using Patient Flow. Learn how to set up high risk waivers HERE.

In HQ, we have automated the process of collecting high risk waivers. You can send the waiver through email and have the owner accept or decline it, or perform a verbal high risk waiver. In this document, you will be able to include the specific reasons for its request.

Steps to Issue an Email High Risk Waiver

  1. Go to Patient Flow > Exam & Premed.
  2. In the ‘Exam’ column, click the Normal button. This button is the path to everything related to the exam of an animal.
  3. Scroll down to the Exam section, click the Request high risk Waiver button.
  4. Select the reasons for requesting the high risk waiver. If the reason is ‘Other’, there is a text box on this screen to specify findings. Once finished, click Done at the bottom of the popup.
  5. Once the high risk waiver has been requested, the patient is put under the HRW tab in Patient Flow. In HRW tab, the reason(s) for the requested high risk waiver are noted under the patient and highlighted in yellow to the right under the Action column.
  6. Select the Email button on the right in the Action column. It is essential to ensure the owner has an email in the Clinic HQ System for this workflow to be successful. You may consider obtaining a verbal high risk waiver if the client does not have an email address available.
  7. The client will sign the waiver to accept the high risk waiver. To decline it, however, the client will select the “Options” button and Decline to Sign.

Note: Your clinic sets up ‘High Risk Waiver Reasons’ in Settings > Medical Notes. They are highly customizable. See the setup guide above on how to do this.

By signing the waiver, the owner accepts the waiver and gives permission to proceed with the surgery. After the owner signs it, the following will happen:

  • Turn the HRW tag green in all places to indicate it was accepted.
  • Upload a copy of the animal’s record in documents.
  • HQ will then remove the patient from the HRW screen.

If the owner declines the high risk waiver, the HRW tag will turn red and the surgery will be declined. HQ will also input the reason for the declined surgery as the Owner declined the High Risk Waiver.
